Ames High to host free sports physical clinic

Ames High Athletics in coordination with the high school nurse’s office will be hosting a free athletic/sports physical clinic on Tuesday, November 8 immediately after the school day in the new Ames High School nurse’s office. 

A signed consent form MUST BE SIGNED BY A PARENT OR GUARDIAN prior to receiving sports physical. Forms are also available to be picked up by students in the nurse’s office during regular hours. Additionally, the front page of the sports physical document should be completed prior to arrival. Extra copies will be available on site, but must be completed prior to the physical.

This will satisfy the physical requirement prior to participating in any athletics program in the coming year. Athletes are required to have one on file every 13 months. This does not substitute a regular annual visit with a primary care physician.

Information: A consent form must be signed by a parent or guardian prior to arriving at the clinic. It will also save time to have the front page of the athletics sports physical completed prior to arriving.
